To request medication from IEPLA, you must follow these simple steps:
Fill out the medication request form through the following link or QR code (you must fill in all the information and attach a photo of the prescription and/or medical report).
Expect a call from our IEPLA pharmacy team within 5 business days to notify you if the medication is available.
If you have your medication, please go to the Social Action coordination office of our church on the day and time indicated by the IEPLA Pharmacy team to pick it up.
